Clinical and Radiographical Observation in Traumatic Acute Abdomen


Abstract
The author studied the total 162 cases of traumatic acute abdomen, who had been visited B. N. U. hospital dating from March 1971 to March 1976, by plain film study of the abdomen.
Radiographical findings were analyzed with the background of definitive diagnosis because all the mate¡©erial observed were done operation.
The results were summarized as follows
1. The sex distribution of total cases was 138 cases(85.2%) in male and 24 cases (14.8%) in females,i . e., the sex ratio 5.8:1(M:F). The most frequent age group was 4th decade (25.3%).
2. Traffic accident was the most frequent causative factor, 50 cases (30¢¥9%), and next orderly by¡© industrial injuries, violence, stab, and fall-down, etc.
In the 4th decade group, the traffic accident and fall-down were the most frequent causativc factor and the 2nd decade, the violence was the most frequent and industrial injuries in teen ages group.
3. Hollow organ injuries were more prevalant than parenchymatous organ injuries. The jejunum and spleen were the most frequent involved organs re¡×pectively.
4. The radiographical findings were demonstrated as belows.
1) Reflex ileus patterns were seen in 100% frequency in hollow organ and urogenital organ damage, 80% in liver and spleen ruptures.
In 20% cases of the parenchymatous organ injuries, no gas shadow was seen in abdominal region.
2) Widening of the interspaces of the each bowel loop was seen in urogenital organ damage in 100% frequency and 80% in hollow organ damage, 50% in parenchymatous organ damage.
3) Free air shadow was seen in hollow organ damage by 25% frequency and 16.7% in urogenital organ damage.
4) Obliteration of properitoneal fat line was seen as much as above 50% frequency in hollow and urogenital organ damage, 27.8% in spleen rupture. The bulged feature of properitoneal fat line were seen 25% frequency in liver rupture cases.
3) In spleen, urogential organs and liver damaged cases, accompanying rib and pubic bone fracture were noted on about 16.2% frequency.
